Do I need a referral?

Referral, Prescription and Prior Authorization requirements vary with each plan.  As such, we always suggest that patients review their benefits prior to treatment.  Your insurance card should have a customer service phone number listed on the back.

Do you accept my insurance?

We accept Medicare as well as most major insurance providers.

Can I be seen if I don't have insurance?

Yes; we offer self-pay treatment.

What should I bring to my appointment?


-Insurance card(s)

-Co-payment (if applicable). We accept Cash, Check or Credit.

-Any referrals, operative notes, MRI/X-Ray reports, or additional information that your referring physician would like us to have.

-Wear comfortable clothing that is easy to move in.  It is best to wear shorts if we are seeing you for your knee and a tank top if we are seeing you for your shoulder.


Our parking lot is located directly across the street from Paint Winni (just after our main entrance with the handicap ramp).
