What to expect at your first appointment-

Please plan to arrive fifteen minutes prior to the start of your first appointment so that we may obtain the necessary paperwork and insurance information to put you in our system as well as give your therapist a chance to get to know your clinical background prior to your evaluation.  Please bring any records from your previous doctor, a list of your medications, allergies and a list of prior doctors.  If you have a current program for therapy, we encourage you to bring this with you as it is helpful to know what you are currently working on.

 

During you initial evaluation, your therapist will work with you to discuss your current status as well as your goals for Physical Therapy.  Based on these factors as well as your personal schedule, frequency and duration of follow-up visits will be determined. 

 

Each evaluation appointment includes a comprehensive history as well as a physical exam to test range of motion, strength, and flexibility.

Your therapist will send an evaluation report to your referring provider as well as your primary care doctor if you have one.  Additional progress notes will also be sent to keep your doctors abreast of the improvements you have made.


 

What should I bring to my first appointment?

  • Your insurance card(s)

  • Your co-payment (if applicable). We accept Cash, Check or Credit.

  • Any referrals, operation notes, MRI/X-Ray reports, or additional information that your referring physician would like us to have.

  • Wear comfortable clothing that is easy to move in.  It is best to wear shorts if we are seeing you for your knee and a tank top if we are seeing you for your shoulder.

  • Feel free to complete and submit the Intake Forms below prior to your appointment.  You can also print the Intake Forms located at the bottom of this page.  Medicare patients will also be asked to fill out a specific functional index form when you arrive.
